As a hardcore gamer who's spent countless hours in Kojima's bizarre post-apocalyptic Australia, I've become obsessed with understanding one of the most unique power systems I've ever encountered in a game. The DOOMS abilities in Death Stranding 2 aren't just cool gameplay mechanics – they represent some of the most creative and thought-provoking supernatural elements I've seen in modern gaming.
For those who don't know, DOOMS is this rare, enigmatic condition that emerged after the Death Stranding event. It's fundamentally tied to the realm of the dead, granting sufferers various supernatural abilities. What makes it so fascinating is how differently it manifests in each character – no two DOOMS abilities are exactly alike!
The one common thread? If you have DOOMS, you're allergic to anything related to the other side. Get near chiralium or BTs, and you'll break out in rashes or start crying. It's like the universe's worst allergic reaction, but with paranormal perks to compensate.

The Hydrologist: When DOOMS Just Makes Life Miserable
Not all DOOMS abilities are badass superpowers – some are just plain terrible! Take The Hydrologist, who vomits every 21 minutes due to her condition. Can you imagine trying to complete a delivery route with that schedule? I'd be rage-quitting every session!
Her body is so attuned to the Beach's tides and tar currents that she can't even eat solid food. While playing through her storyline, I couldn't help but feel immense sympathy for her predicament. She studies water from the Beach alongside Heartman, but at what personal cost?
Interestingly, both she and her daughters (the Chronobiologist and Metagenomicist) all suffer from different 21-minute syndromes. I found it fascinating that they share the likenesses of Nicolas Winding-Refn's family – the same director whose appearance was used for Heartman. These meta-connections are what make Kojima's storytelling so uniquely layered!
The Doctor: Two Right Hands Make Everything Right
When I first encountered The Doctor in-game, I literally did a double-take. Two right hands? Are you serious? But in the twisted logic of Death Stranding 2, it makes perfect sense.
Played by the talented Debra Wilson, The Doctor leads The Motherhood sanctuary in Australia. Her DOOMS ability allows her to reach into a Stillmother's womb and extract the fetus with her two hands, placing it directly into a BB pod. No C-sections or surgical tools needed! How incredible is that?
I remember watching her help Rainy check on her baby – the animation was so beautifully unsettling. It's these moments that make me appreciate how Kojima blends the bizarre with the profound. Isn't it fascinating how DOOMS can manifest as something so specifically useful in this broken world?
Dollman: My Favorite Traveling Companion
I'm not ashamed to admit it – Dollman became my favorite character during my playthrough. This chatty doll isn't just comic relief; he's got a tragic backstory that hit me right in the feels.
Before becoming a doll, his DOOMS ability allowed him to communicate with the dead like a medium. He died trying to help his daughter see her deceased mother, and in the process, replaced his daughter's soul with his own, trapping his Ka (soul) in her doll. How messed up is that?
While playing, I found his constant chatter during long delivery routes both comforting and informative. Even though he lost his original DOOMS abilities, he still maintains a connection to the other side, helping me sense BTs before they could drag me into those terrifying encounters. A true MVP in my book!
Heartman: Death Every 21 Minutes
Heartman's condition has to be one of the most fascinating and limiting DOOMS manifestations in the game. His heart stops every 21 minutes, making him clinically dead for three minutes before his AED kicks in to resuscitate him.
What I find most compelling about Heartman is how he's adapted to his condition – even embraced it! He uses those three minutes of death to search for his family's Beaches. His entire living space is designed around his 21-minute cycles. Talk about making lemonade from extremely weird lemons!
I was genuinely moved when I learned that Deadman donated his heart to Heartman before passing. These moments of humanity amid the supernatural chaos are what make Death Stranding 2 so special to me. Wouldn't we all make similar sacrifices for those we care about?
Tarman: Navigating the Impossible
Tarman quickly became one of my favorite new additions to Death Stranding 2. As the pilot of the DHV Magellan, his DOOMS ability is both practical and visually striking.
By inserting his arm stump into the nav console, he can sense the tar currents and navigate through the black tar that would be impossible for others to traverse. The first time I witnessed this in action, I was blown away by the visual and the concept.
His backstory – losing his hand and son to the tar currents – adds a poignant dimension to his abilities. The psychic connection he maintains with his lost hand (and by extension, his son) creates this beautiful metaphor about holding onto what we've lost. I may have shed a tear or two during his storyline, not gonna lie.
Rainy: Weather Control with a Twist
Rainy's ability to summon Timefall wherever she goes initially seems like a curse – after all, Timefall rapidly ages everything it touches. But the twist that her immediate vicinity creates "corefall" that reverses aging effects? Absolutely brilliant game design!
Playing through her story arc was particularly satisfying. Seeing her transform from an outcast accused of witchcraft to a key member of Drawbridge with the power to potentially heal the world gave me all the feels. The moment she healed Fragile's withered skin was one of the most beautiful sequences in the game.
Her abilities represent hope in a world desperately lacking it. Isn't it interesting how something initially perceived as destructive can become the key to salvation? Classic Kojima storytelling!
Tomorrow: The Deadliest Child
Tomorrow is hands-down one of the most intriguing characters in Death Stranding 2. A child with the power to produce tar from her body and rapidly age everything she touches? That's nightmare fuel wrapped in an innocent package.
Her ability to move freely through tar makes for some of the most visually stunning combat sequences in the game. I still remember the first time she saved Sam by emerging from the tar and disintegrating ghost mechs – I literally jumped out of my chair!
The revelation that she is actually Lou and Sam's daughter explains her high DOOMS level, but it doesn't make her any less terrifying. How do you parent a child who can literally age you to death with a touch? These are the philosophical questions that keep me up at night after playing this game!
Higgs: The Ultimate Hater
Higgs remains one of gaming's most compelling villains, and his enhanced DOOMS abilities in Death Stranding 2 make him even more threatening.
His ability to summon and control BTs, teleport, and now control ghost mechs makes every encounter with him a nail-biting experience. What I find most fascinating is that his powers weren't innate – they were bestowed by Amelie, making him a manufactured villain in many ways.
The time he spent trapped on the Beach between games has clearly driven him further into madness. His obsession with revenge against Fragile and Sam creates this perfect storm of hatred and power. Is there anything more dangerous than a superpowered villain with a grudge? I think not!
Fragile: Not So Fragile After All
Fragile's "jumping" ability remains one of the most practical and visually striking DOOMS manifestations in the game. Being able to teleport across vast distances by slingshotting through the Beach? That's a delivery person's dream!
What makes her character arc so compelling is the "jumpshock" condition she develops – her body literally rebelling against her power usage. It creates this perfect metaphor for the cost of our abilities and the sacrifices we make to help others.
I appreciate how Death Stranding 2 shows her powers being developed into technology via transponders. It's a clever way to maintain the fast travel mechanic while acknowledging her character's limitations. Gameplay and narrative harmony at its finest!
Sam Bridges: Death Is Just the Beginning
Finally, we have our protagonist Sam, whose repatriation ability fundamentally changes how we approach death in games. His ability to return from death makes him the perfect video game protagonist – his mechanics literally match the "die and try again" nature of gaming!
As the first person to ever suffer from DOOMS and the first BB, Sam's connection to the Death Stranding runs deeper than any other character. His effective immortality creates fascinating story implications – what does it mean when death isn't final? How does that change how you approach danger and relationships?
The limitation that his soul can never reach the Beach creates this poignant existential dilemma. Is immortality a gift or a curse? After spending 100+ hours with Sam, I'm still not sure I know the answer.
Final Thoughts: DOOMS as Metaphor
After spending countless hours with Death Stranding 2, I've come to see DOOMS as more than just cool superpowers. They're metaphors for how we adapt to trauma, how disabilities can become unique strengths, and how connection – even to something as terrifying as death – can be transformative.
Each character's DOOMS manifestation reflects something about their personality and past. Isn't that true of our own abilities and limitations? We are shaped by our experiences, and in turn, we shape our world with what we've become.
